Include the essentials
A referral letter with the diagnosis, relevant history and any precautions. Surgical and funding details help us plan the first session.
We take it from there
We contact your patient to arrange an appointment and keep you informed as treatment progresses.
What to include
Helpful details for a smooth start.
Nothing here is mandatory. If some details aren't to hand, send what you have and we'll follow up.
- Patient name, date of birth and contact number
- Diagnosis and mechanism of injury
- Date of surgery and the operation report, if applicable
- Surgeon's protocol, precautions or restrictions
- Relevant imaging reports
- Funding details: care plan, TAC or WorkSafe claim number, DVA or NDIS details
Funding pathways
- Medicare: GP referral under an eligible care plan
- TAC and WorkSafe: accepted claim number
- DVA: referral where applicable
- NDIS: self-managed and plan-managed
- Private health and self-funded patients
